Volunteer Activity Certificate Issuance Procedures

The procedure for issuing a certificate of volunteer activity at APCC is as follows:
It will take about a week to deliver.Therefore, we ask that you submit your application form as soon as possible.

1. Please download the application form from this screen.

*When downloading "★Excel", an error may occur if you do not download it from a PC.

2. Fill in the necessary information in the downloaded application form,Please submit your application to APCC immediately by email (volunteer@apcc.gr.jp) or fax (092-734-7711)please.
【Notice】
If you create it in Excel and attach it via email, you can leave fields 13 and 14 (signature/seal) blank, but please print it out and sign it on 13 and 14 by the date you receive the activity certificate.・Please be prepared to submit the stamped version.
If you have filled out all the fields by hand, you may take a photo of the completed application form and attach the photo to an email. Please make sure that the text is clearly visible when taking the photo.

3. Once the issuance is complete, we will contact you, so please come to the APCC secretariat.
