About activity certificate

If you need proof of your participation in APCC volunteer activities for high school/university entrance exams, job hunting, etc., please submit the application form by following the steps below.It will take about a week to create it.We appreciate your time, so please submit the application form well in advance.In addition, 500 yen per letter will be charged for the issuance of the activity certificate..

【Notice】
If you create it in Excel and attach it via email, you can leave fields 13 and 14 (signature/seal) blank, but please print it out and sign it on 13 and 14 by the date you receive the activity certificate.・Please be prepared to submit the stamped version.
If you have filled out all items by hand, you may take a photo of the completed application form and attach the photo data to your email.
Please make sure that the text is clearly visible when you take the photo.
2. Please fill out the necessary information on the downloaded application form and immediately submit it to APCC by email (volunteer@apcc.gr.jp) or fax (092-710-6103).

3. [Confirmation certification]:APCC office will send it to you via email.
*Please be sure to check that all contents are complete.
4. After confirming the certificate,Please reply to the APCC office.Also, in your reply email, be sure to include the date and time you would like to receive it at the APCC office.

*Please note that the certificate will be issued only after receiving your reply.
5. Please come to the APCC office on the pick-up date and time.

If you are unable to come to the APCC office, such as because you live far away, you can also send the application by mail.
If you would like to receive your application by mail, please check the following and apply.

2. After filling out the necessary information on the downloaded application form, please be sure to enclose a return envelope with an 84 yen stamp and mail it to the APCC Office.
*Please also write your name and mailing address on the return envelope.
3. Please transfer the issuance fee (500 yen). The certificate will be mailed after the transfer is completed.
